Qualifications: Educational preparation includes, at minimum, a graduate of a
regionally accredited or New York State registered college or university with
an LMSW

Experience: 5+ years of experience in related field

Summary: The Manager of Care Coordination coordinates and manages the SW/CM
services of the hospital assuring all patients receive quality services. Is
accountable and responsible for the overall management of the department s
human, fiscal and material resources as well as the integration of SW/CM
services for patients. Represents SW/CM in the planning of multi-disciplinary
and inter-departmental programs. Participates with other programs or department
directors in policy development and strategic planning for Columbia Memorial
Hospital as they relate to patient care. Manages all aspects of SW/CM and
ensures staff adherence to policies, procedures and processes related thereto.

Salary Range: $28.06 – $51.36
