Education: Graduate from accredited school of nursing with current licensure
and registration as a Registered Nurse in the State of New York preferred

License/Certifications: Registered Nurse (RN) preferred
Certified Diabetic Educator, preferred

Experience: Experience providing patient education and counseling services.

Summary: Provides educational services to clients, medical staff and community
members on all aspects of diabetes and diabetes self-management techniques.

Duties: Provides individual and/or group counseling on all
aspects of diabetes self-care/management, including meal planning, physical
activity, hypoglycemia, medications, self-glucose monitoring and use of
results, insulin administration, prevention of complications, life-style
changes, goal-setting, general hygiene, foot care, dental care, insulin pump
therapy, family involvement and social support, and psycho-social adjustment.

Assess client s learning needs, abilities, preferences and readiness to learn
prior to giving education.

Performs blood glucose monitoring and blood pressure checks.

Instructor for one or more sessions in each educational class

Participates in curriculum development and revision.

Develops education programs that consider client s cultural and religious
practices, emotional barriers, desire and motivation to learn, physical and
cognitive limitations, language barriers, and the financial implications of
care choices.

Documents/keeps records up to date in client medical records and/or EMR
adhering to established policies and procedures.

Communicates client s progress to referring physician.

Participates in diabetes education for medical professionals and support staff
in the community as requested

Provides individual and/or group education on related health topics as directed
by the Medical Director of the Care Centers.

All other duties as assigned by supervisor
