At CMH we make comfort and personal care our priority as we guide patients and their families through their end-of-life journey. The Community Hospice works closely with healthcare professionals at CMH to provide additional expertise, help and support to seriously ill patients.
In most cases, patients cared for by Community Hospice leave the hospital to return to their own homes, where they can have the best possible quality of life, surrounded by the people and things they love. Hospice team members visit the patients at home, providing outstanding medical and palliative care to the patients themselves, as well as advice and guidance to their family members. Care is also provided to patients in area nursing homes and at Columbia Memorial Hospital. The team is available 7 days a week, 24 hours a day, if a patient or family member needs help.
